For Illinois patients:

When we have vaccines available, we will notify you through our online patient portal called MyChart to schedule an appointment. It's free and simple to register for a MyChart account at HSHS. MyChart is a helpful tool for patients at all local HSHS facilities, including our hospitals and clinics.

Please follow the instructions below to download and/or register for MyChart.

1. Register for MyChart or Log into MyChart
2. Click on the Scheduling Invitation
3. Select an appointment date and time
4. Complete the e-check in process in MyChart
5. Arrive at the vaccination location at your scheduled time

Log into MyPrevea or create a MyPrevea account.

Why should I get the COVID-19 vaccine? 

Receiving the COVID-19 vaccine is the best way to combat the disease. There is a light at the end of the tunnel and we need all the help we can get in fighting COVID-19. That is why we continue to ask you to wear a mask and social distance as well as get your COVID-19 vaccine when it becomes available to you.

Do I need an appointment to get vaccinated? 

Yes. When and if COVID-19 vaccines are available at your local HSHS facility, they will be available by appointment only through MyChart in Illinois and MyPrevea in Wisconsin. We are not accepting walk-ins at this time.

For the most accurate information, please select your nearest location from the list above. As we make any adjustments to our distribution plans, we will update the website accordingly.
