A certified application counselor will be available at HSHS Good Shepherd Hospital to help individuals and families sign up for health care coverage through the Affordable Care Act. To enroll, individuals have until December 15, 2017.
Enrollment opportunities will be available on one day only, Saturday, December 9, 2017, from 8 a.m. – 3 p.m. Please use the east entrance, 200 South Cedar Street, Shelbyville.
Those planning on attending should be prepared to provide full names, birth dates, and Social Security numbers for all persons enrolling; proof of Illinois residency (i.e. Illinois driver’s license, state I.D, etc.); and employer/income information (i.e. paycheck stubs, W-2 or other information about your family’s income).
