Requirements for Hospice Admission

 
  • Diagnosis of a terminal illness for which curative treatment is no longer sought.
  • Patient and family members must be aware of the diagnosis and prognosis and wish to receive hospice care.
  • Order of the attending physician.
  • A designated caregiver/family member must be available when needed.

Hospice Services

 
  • 24-hour a day pain management and support.
  • Assistance with emotional and social stresses that surround terminal illness.
  • Help with daily activities such as bathing, grooming, dressing and hair care.
  • Spiritual support and counseling. However, families always have the option to utilize their own spiritual support system.
  • Respite care for family members and loved ones, including help with shopping or errands, as well as companionship and support.
  • Bereavement counseling and support for loved ones for up to 13 months after death.
  • Grief support groups in Springfield and Decatur.
