(Effingham, IL) – The Community Connected Care Collaborative, in partnership with HSHS St. Anthony’s Memorial Hospital, invites members of the greater Effingham community to join them for a public program on Wednesday, April 4, called, “Dementia, Advanced Directives and Transitions of Care: How to Prepare for the Future.”  This program, held from 11 a.m. to 1 p.m. in HSHS St. Anthony’s Auditorium, will discuss these topics and answer your questions about long-term and future planning.
 
The event will include presentations by Kelly Willenborg called “Mind by Design”; Ashley Dillingham, MSSW, LSW, HSHS Home Care and Hospice Southern Illinois, about “The Hardest Conversation”; and a Transitions of Care Panel.  The event will also feature informational booths and refreshments.
 
The Community Connected Care Collaborative is a joint venture between HSHS St. Anthony’s Memorial Hospital and nursing facilities, physician’s offices, and community agencies in the greater Effingham area. The purpose of the Collaborative is to discuss opportunities and implement strategies which enhance patient care across the health care continuum.
