Certified application counselors will be available by appointment only at HSHS St. John’s Hospital to help individuals and families sign up for health care coverage through the Affordable Care Act. Individuals and families have until December 15, 2018 to enroll.

Enrollment opportunities will be in the Yellow Rose Tea Room, off the lobby of HSHS St. John’s Children’s Hospital, on the following dates:

November 30, 2018: Noon - 9 p.m.
December 15, 2018: 9 a.m. - 5 p.m. 

When preparing for the event, please bring Social Security numbers as well as first and last name for all persons enrolling, proof of Illinois residency (Illinois driver’s license), and employer/income information (paycheck stubs, W-2).
 
Those interested in attending should call Mary Rogers at (217) 814-4308 to schedule your appointment. Be prepared to provide your name, birth date, address, income information and Social Security number when scheduling your appointment.
