Certified application counselors will be available by appointment only at HSHS St. John’s Hospital to help individuals and families sign up for health care coverage through the Affordable Care Act. Individuals and families have until December 15, 2019 to enroll.
Enrollment opportunities will be in the Women and Children’s Clinic on 400 N. 9th St. (across from HSHS St. John’s Children’s Hospital). Enter through the main lobby off Ninth Street on the following dates:

When preparing for the event, please bring Social Security numbers as well as first and last name for all persons enrolling, proof of Illinois residency (Illinois driver’s license), and employer/income information (paycheck stubs, W-2).
