HSHS St. Joseph’s Hospital Breese is working with AgeSmart Community Resources to offer a free, chronic disease self-management workshop starting June 5. The Live Well Be Well series is designed to help attendees feel better, live healthier, improve quality of life, and maintain independence.
 
Sessions are based on Stanford University’s Chronic Disease Self-Management Program. The focus is to teach proven methods of: controlling pain and other symptoms; communicating more effectively with family, friends and health professionals; managing medications; improving strength, flexibility and endurance; and healthier eating.
 
This series will take place six consecutive Monday afternoons from 1:00 to 3:30 at the Villas at St. James. Those dealing with a chronic condition are encouraged to attend. Examples of those who could benefit include people dealing with arthritis, heart disease, diabetes, chronic pain, hypertension, COPD, stroke, asthma, or any other challenging condition.
