Our service area includes Chippewa and Eau Claire counties plus portions of: Barron, Dunn, Pepin, Buffalo, Trempealeau, Jackson, Clark, Taylor and Rusk counties. Referrals are accepted based on the location of the patient’s home and our team’s ability to meet each patient’s unique needs.

Lifeline® Medical Alert System


Lifeline is a personal medical alert response system designed for those with ongoing medical conditions, mothers with chronically ill children, and elderly or disabled persons who want to live in their own homes.

Several models and options are available based on specific needs and lifestyle, including fall detection that automatically calls for help even if the patient is disoriented or unconscious and cannot push the button and wireless models that can be activated outside of the home.

The Best Plan of Care


Our Home Health professionals work closely with each patient, their family and their physician to develop the best plan of care to meet individual needs. Medication review, education, skilled assessments and treatments are essential to recover from a hospital stay or illness. Therapy can be provided to regain strength and ensure a safe adapted environment.
