Who is eligible?

 
  • Adults 18 years and older who are underinsured or uninsured for dental care.

  • Adults with an emergency need for tooth extraction.

  • Adult residents of Macon County. 

Coverage is provided for two dental exams with X-ray, one extraction and up to three fillings per year.

How does the program work?

 
  • A patient is referred to Catholic Charities from a physician, dentist, social service agency or by self-referral.

  • The patient meets with Catholic Charities to determine his or her eligibility for the Dental Voucher program.

  • Once the patient's eligibility is established, Catholic Charities will contact a participating dental office and provide the patient with a dental voucher.

  • The patient will make an appointment with the dentist for an initial exam and X-ray.

  • The voucher must be presented at the time of the dental appointment.

  • If the dentist refers the patient to an oral surgeon, the patient must present the voucher at surgery appointment.

Participants will be asked to complete a survey regarding the program following their final appointment.

HSHS St. Mary's Hospital has identified access to health care services as one of the top identified needs in Macon County. A Community Health Needs Assessment   indicated a lack of access to affordable dental care in Macon County and the surrounding area.
