How does the program work?

A patient is referred to Catholic Charities from a physician, dentist, social service agency or by self-referral.

The patient meets with Catholic Charities to determine his or her eligibility for the Dental Voucher program.

Once the patient's eligibility is established, Catholic Charities will contact a participating dental office and provide the patient with a dental voucher.

The patient will make an appointment with the dentist for an initial exam and X-ray.

The voucher must be presented at the time of the dental appointment.

If the dentist refers the patient to an oral surgeon, the patient must present the voucher at surgery appointment.
Participants will be asked to complete a survey regarding the program following their final appointment.
