  • Restorative Nursing Programs - Participate in customized exercise designed to maintain and improve independence with activities of daily living.

  • Personal Care and Assistance - Specialized assistance focused on your unique daily needs, including bathing, grooming, dressing, etc.

  • Activities, Including Outings - Staff assists clients with various activities ranging from social to purposeful. Our goal is to promote independent living.

  • Medication Reconciliation - Our staff will work as a liaison between your physician and pharmacy to help maintain a proper medication regiment.

  • Regularly scheduled visits with the children from Decatur Day Care


  • Door-to-Door Service - St. Mary's Adult Day Services proudly offers transportation to and from the program. Our competent and caring drivers can transport you in our handicapped accessible busses.

  • Medical and Outpatient Appointments - Our staff will happily assist you to your medical appointments on days that you spend with us at the Day Services Program on the St. Mary's Hospital Campus. This includes outpatient hospital appointments, as well as general physician follow-up appointments.

Service Coordination

  • Outpatient Services - We can help to schedule your therapy and physician appointments.

  • Development and Management of Individualized Care Plan - including, but not limited to: medication reconciliation, assessments by a licensed nurse, communication with your attending physician and skilled nursing services.
