In collaboration with the patient/family, social workers, physicians, interdisciplinary team, and payer representative, the Care Manager-RN ensures patient progression through the continuum of care in an efficient and cost effective manner. Identifies, initiates and manages optimal care coordination and discharge planning to develop patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety and length of stay management. Provides support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Monitors the provision of cost effective care to ensure care is given in the most clinically appropriate setting. Facilitates compliance with regulatory requirements and organizational performance standards, including demonstration of core competencies.

Summary:

HSHS St. John’s Hospital is seeking a proactive and compassionate Registered Nurse (RN) Case Manager to join our interdisciplinary care team. This role is essential to supporting patients throughout their hospital stay and transition of care by coordinating clinical services, assessing discharge needs, and ensuring optimal outcomes through timely and effective case planning.

Key Responsibilities:

  • Conduct patient assessments to determine care coordination and discharge planning needs
  • Collaborate with physicians, nurses, social workers, and outside agencies to support patient goals
  • Monitor patient progress and facilitate timely transition across levels of care
  • Ensure compliance with hospital policies and regulatory guidelines
  • Advocate for patient needs while promoting efficient use of healthcare resources

Education Qualifications

  • Associate’s Degree or Diploma in Nursing is required.
  • Bachelor’s Degree in Nursing is preferred.

Experience Qualifications

  • 2 years of clinical experience is required.
  • Prior case management experience is preferred.
  • Quality/Process Improvement experience is preferred.

Certifications, Licenses and Registrations

  • License as a Registered Nurse (RN) in the state of practice is required. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.
  • Case Management certification (ACM, CCM, RN-BC) is preferred.
  • Basic Life Support Certification (BLS) is required within 60 days of hire.
