Our service area includes Chippewa and Eau Claire counties plus portions of: Barron, Dunn, Pepin, Buffalo, Trempealeau, Jackson, Clark, Taylor and Rusk counties. Referrals are accepted based on the location of the patient’s home and our team’s ability to meet each patient’s unique needs.

Specialized Medical Care

 

Palliative care is specialized medical care that includes emotional and spiritual support, along with relief of physical symptoms. Whether the diagnosis is curable, chronic or life-threatening, our purpose is to improve quality of life for both the patient and the family. The palliative care team is a specially-trained team of doctors, nurses, social workers, and other specialists who work together with a patient’s doctors to provide an extra layer of support. It is appropriate at any age and at any stage in a serious illness.

Home-based palliative care services can be helpful for those who have experienced several trips to the emergency room or admissions to the hospital because of their illness, and particularly for those who have increased difficulty managing and understanding their care.

These services are provided within the home – wherever home may be: residential, nursing home, or an assisted living facility. Medicare, Medicaid, and some insurance companies will pay for home-based palliative care.

Grief Support

 

Death is one of the most challenging human experiences. It leaves an emotional wound that takes time and expression of grief to heal. The Chippewa County Grief Education Guidance Team provides an ongoing Grief Support Group which offers a safe place to listen or express thoughts without rejection or judgment. The group is free, open to all, and meets the 2nd and 4th Monday of each month from 6:30 to 8 PM at our hospital. Please call 715-717-7581.
