What We Do

Our interdisciplinary home care team includes advance nurse practitioners, registered nurses, certified nursing assistants, and licensed social workers.

Our Services

  • Skilled nurses who evaluate, monitor and teach you about your medical condition and administer IV medication, tube feedings and provide wound care.
  • Home health aides provide personal care (such as bathing and grooming), assist with therapy exercises and help more health nurses and therapists provide patient care.
  • Physical therapists use exercises and special equipment to help reduce pain and restore mobility, strength and balance.
  • Occupational therapists help patients overcome or live with disabilities by teaching them to use special techniques or equipment in the home to perform tasks like bathing, cooking, dressing, etc. 
  • Speech-language therapists will evaluate and develop a treatment plan to improve speech, language, processing and swallowing.
  • Social work services provide patient counseling and referrals to community agencies for caregivers, financial assistance and transportation.
  • Patient and caregiver education is in every treatment plan and is a very important part of home health care.

Who Qualifies For Home Health?

The Centers for Medicare and Medicaid Services states that for a patient to be eligible to receive covered home health services, a physician must certify that the patient is confined to his or her home. An individual is considered “confined to the home” (homebound) if he or she:

  1. Because of illness or injury, needs the aid of supportive devices such as crutches, canes, wheelchairs and walkers; the use of special transportation; or the assistance of another person in order to leave their place of residence
  2. Have a condition such that leaving his or her home is medically contraindicated.

In addition, there must exist a normal inability to leave home, and leaving home must require a considerable and taxing effort.

Please note, an individual may still qualify for home health care if he or she leaves home for medical treatment or short, infrequent absences for non-medical reasons, such as attending religious services.
