Introduction

As a ministry of the Hospital Sisters Health System, our mission at HSHS Home Care & Hospice is to reveal and embody Christ’s healing love for all people through our high-quality Franciscan healthcare ministry. We promise to provide exceptional care in your home environment. 

Activities are directed toward helping you to attain and maintain your highest level of independent living, and to help your family support your health journey. We will meet with you upon admission to discuss your goals and your physician’s goals for your health. Efforts are also concentrated on the prevention of further disability, health teaching, and on appropriate referral to allied health professionals as needed.

What we do

Our interdisciplinary home care team includes advance nurse practitioners, registered nurses, certified nursing assistants, licensed social workers, physical therapists, occupational therapists and speech language pathologists.

Our Services

  • Skilled nurses who evaluate, monitor and teach you about your medical condition and administer IV medication, tube feedings and provide wound care.
  • Home health aides provide personal care (such as bathing and grooming), assist with therapy exercises and help nurses and therapists provide patient care.
  • Physical therapists use exercises and special equipment to help reduce pain and restore mobility, strength and balance.
  • Occupational therapists help patients overcome or live with disabilities by teaching them to use special techniques or equipment in the home to perform tasks like bathing, cooking, dressing, etc. 
  • Speech-language therapists will evaluate and develop a treatment plan to improve speech, language, processing and swallowing.
  • Social work services provide patient counseling and referrals to community agencies for caregivers, financial assistance and transportation.
  • Patient and caregiver education is in every treatment plan and is a very important part of home health care.
