Step-by-Step Guide to Completing Requirements

1. Complete online Health Risk Assessment (HRA), CLICK HERE

Username

Employee: City of Springfield username (example:  bjohnson)
Spouse: Employee username + “s” (example:  bjohnsons)

 Password

Returning users: Enter password from prior Wellsource registration

New users: Enter password provided by City of Springfield Human Resources (Prompting to change password will occur upon registration)
 

2. Complete annual biometric screening

CLICK HERE to download the biometric form PDF to be completed by provider of choice (subject to normal billing processes of your provider); this form must be returned to the LeadWell clinic or faxed to 217-529-4228

Health Risk Assessment

What is a Health Risk Assessment (HRA)?

The HRA is a confidential online questionnaire to assess general wellness in a variety of categories to highlight areas of strength and improvement, as well as provide areas of focus to increase overall wellness. 

What is a biometric screening?

A biometric screening is a preventive health screening that assesses cholesterol, glucose, blood pressure, waist circumference, and body fat percentage.

What are the benefits of completing the biometric screening with LeadWell’s Certified Health Coach?

  • In-depth review of results and consultation with guidance toward living a healthy lifestyle
  • Performed at no cost to the patient
  • Laboratory testing is performed via finger-stick with rapid results on-site
  • Year-over-Year comparison of prior results
  • Referral program partnership with FitClub (physical activity/nutrition programs), Joslin Diabetes Center (Diabetes Education), and Prairie Heart (Cardiac Consultation)

Am I required to fast before my biometric screening?

You are not required to fast before your biometric screening. However, it is recommended that you fast about 9-12 hours prior — refraining from food and drink with the exception of water, unsweetened tea or black coffee.
