Community AED Program

Prairie Cardiovascular is the local leader in heart health throughout central and southern Illinois. We want to ensure our community members in the regions we serve have life-saving tools and education to provide immediate assistance to those in need. When treating a heart attack, every second matters. Research proves Cardiopulmonary Resuscitation (CPR) and Automated External Defibrillators (AED) save lives. In partnership with Prairie Heart Foundation and the McGraw Family, we are proud to provide the Community AED Program to organizations in the communities Prairie serves.

Community AED Program Application

Please complete and submit the application form below.

Organization must provide 501(c)(3) determination, social service agency verification or proof the requesting entity is who they say they are.
I agree for the agency/organization's staff and volunteers to participate in a mandatory training session to learn how to use the equipment by a Prairie Heart representative.
I agree to participate in a donation presentation where photographs, video and/or audio may be obtained for publicity purposes
I understand that the Automated External Defibrillator and associated equipment ("AED") is being conveyed as is, where is, and Donor makes no warranty, either express or implied, as to the condition of the AED, or as to the suitability of such AED for Donee's intended purpose or use, nor does Donor make any representation or warranty of any nature whatsoever with respect to AED. Donee assumes sole responsibility and liability for the use of such AED and expressly assumes any and all risks of injury or damages arising from or relating to the AED. Donor expressly disclaims any liability for any injuries or accidents that Donee has from the use of the AED.
