To request a copy of your medical records from our Health Information Department , download and complete the Medical Records Release Form. Then complete the authorized request through the following options:
Call or visit our Health Information Department at 618-234-2120, ext. 11742. We are open from 8 a.m. to 4:30 p.m., Monday through Friday. The Health Information Release of Medical Records office is located just inside the Outpatient Entrance on the first floor near the Registration desk. A photo identification will be requested to release copies of medical records.
