Transfer a patient

What happens when you call AccessOne?

  • Your call will be answered promptly, 24/7
  • You will speak with an experienced RN
  • You provide brief details on the purpose of your call, including:  
    • Service requested
    • Patient demographics
    • Summary of patient presentation
  • Your call will be connected via three-way call to a physician or the appropriate specialist
  • You are asked to provide a financial sheet to aid the registration staff in pre-admission of the patient
  • We will provide you with room assignment and telephone number for the nurse-to-nurse report
  • The referring facility is required to arrange for medical transport

Please have the following information ready

About You:

  • Facility/provider name
  • Provider call back number
  • Reason for call: transfer, consult, direct admit, other
  • Service requested

About the Patient:

  • Patient’s legal name
  • Date of birth
  • Sex/gender identification
  • Brief description of injury or illness

Please send the following items with the patient or transferring agency at the time of transfer: 

  • Medical history and physical
  • Nursing record with current vital signs
  • Copies of all pertinent diagnostic studies
  • Laboratory results
  • Medication history
  • Signed “Release of Information” form
  • Advanced directives, if available 

To best facilitate continuity of care, please fax the following information to the number provided by your AccessOne nurse: 

  • FACE sheet
  • Copy of patient insurance card (both sides) and HMO authorization if a non-emergent transfer
