Advance Directives and DNR Orders Include:

 
  • Health Care Power of Attorney (Naming a trusted relative or friend to communicate for you)​

  • POLST/DNR Order (This order reflects the wishes of someone who is seriously ill or with a life-limiting illness. It states that cardiopulmonary resuscitation (CPR) will not be attempted if the heart or breathing stops.)

  • Health Care Surrogate (If you do not have a Health Care Power of Attorney, a "surrogate" or decision maker will be chosen for you under Illinois Law. )
