How to request a copy of your medical records for you or your dependent.

Medical record requests

Step 1

Download and complete our Authorization Form.

Step 2

Then complete the authorized request through the following options:

  • Call or visit our Health Information Department at 618-526-5405. We are open from 8 a.m. to 4:30 p.m., Monday through Friday. A photo identification will be requested to release copies of medical records.
  • Fax a request or authorization form to 618-526-4537.
