The first visit at the pain clinic will include an initial evaluation, medical history questionnaire, interpretation of tests and x-rays, and a physical examination. The practitioner will then discuss the recommended plan of treatment. Most injections are done with x-ray guidance for precision and requires pre-scheduled appointment. Please note: You may or may not receive an injection at your first appointment.
What you should bring to your first appointment:
- Completed and signed Intake Form
- A detailed list of your current medications or the pill bottles and a list of medications covered by your insurance company. This enables us to obtain correct dosages and instructions of your medications.
- Photo ID and Insurance Card.
Our physicians may refer to other specialists for the treatment of your pain. Typical specialists involved in the evaluation and treatment of pain sometimes include, but is not limited to: 1) Neurologist, 2) Psychologist/Psychiatrist, 3) Neurosurgeons, 4) Orthopedic Surgeon. These referrals will be at the discretion of the pain physician treating you. If you are referred to another specialist, your continued treatment in the Center for Interventional Pain Management will require your compliance with these referrals.
Please notify the center if you cannot keep an appointment. This allows us to fill the spot for someone else. If you do not show up or call to cancel your appointment, you will be charged a $25.00 fee (appointments canceled less than 24 hours will be considered a “no show”). Please note: repeatedly missed appointments may be cause for discharge from the Center.
