The Almost Home program at HSHS St. Joseph's Hospital is a short-term rehabilitation program designed to help patients transition from hospital care to home.

Through a combination of physical and occupational therapy, attentive nursing care and medical supervision, the program helps patients regain strength, restore mobility, and safely return to everyday life.

Qualified patients who have been admitted to HSHS St. Joseph’s Hospital in Breese or at other facilities for acute care can transfer to our Almost Home Program for their rehabilitation phase of recovery.
