St. Vincent participates in community health needs assessments with the objective of seeking community input to identify and prioritize health issues that the hospital is best positioned to address in collaboration with the community.

HSHS St. Vincent Hospital has a long history of collaborating with the Brown County Public Health Department to plan for our community‘s future health needs. We worked with the Brown County and City of DePere Health Departments to identify Brown County’s top health issues through the Community Health Improvement Process (CHIP). Representatives from HSHS St. Vincent Hospital served on both the steering committee for this project and the Community Partner meetings where the priorities were identified. Action plans were developed by identifying community strengths, existing resources and gaps.
