2025 Tribute
Due to our on-going construction project within the hospital, we cannot host an in-person gathering. This year's tributes will be published in a December issue of the Decatur Tribune. Those wishing to honor an important person in their life can complete the form below or pick up a tribute form at the hospital information desk. Submissions must be received by Nov. 25, 2025 to be included in the Decatur Tribune.
